  • Can I make a same day appointment or just walk in?
    There are a limited number of same-day appointments available each day; call our office for availability. We recommend that you call or come to the health center to make an appointment; we will do our best to accommodate walk-ins.

    At Livingston Community Health, we are committed to treating and using your protected health information responsibly. Our Notice of Medical Information Practices is available at our health centers, it describes the personal information we collect and how we use or disclose that information.
  • What should I bring?
    -Medi-Cal - Medicare or a copy of your medical health plan card - Immunization records - All medication containers - Bring your I.D. or other form of identification (if you have one)
  • How can I pay for my visit?
    Livingston Community Health accepts plans from Medi-Cal, Medicare, Covered CA, and most insurance programs, as well as self-pay. Our self-pay sliding fee discount program is also available based on family size and income. We also offer free application assistance if you need help paying for your visit. Just bring us a government-issued photo ID and pay stubs or your most recent tax return.
